One piece. There is no minimum lot at inflpanjlott.com. The first run costs more per piece than the second because the fixture has to be made either way, but we would rather do your single prototype than tell you to go and buy forty more so the maths works.
0.01 mm flatness on plates up to 900 mm, Ra 0.2 µm on electropolished stainless, 60 to 120 micron powder film. Those are the standard numbers. Anything tighter is a special job and we quote it as one rather than pretending it is standard.
Quote in one working day, first article in 48 hours, then three to nine days depending on the route. Hard chrome is the slow one at six to nine days, because the plating chemistry sets the pace and pretending otherwise is how you get pitted chrome.
Yes. inflpanjlott.com runs scheduled pickup and delivery routes nationwide, and rush lots travel overnight. If you already have a carrier, we receive and return through the same door. Freight is a line on the quote, never a surprise on the invoice.
Lot identification, the readings we took and where, the process parameters used, the operator who ran the job and the date. Roughness, flatness, film thickness or plating thickness depending on the route. If your quality system needs something specific on it, tell us at the quote stage and we will build the sheet around that.

Both. About half our work comes from companies with fewer than fifty people, and a single prototype gets exactly the same attention as a forty-thousand-piece batch. The only thing that changes is volume pricing, never the route or the documentation.
Describe what you are trying to achieve rather than the process name. A customer asked for a "chrome" effect on aluminium, which does not work; what they actually needed was a bright passivated finish that matched a stainless fitting, and that took one phone call to sort out.

The question nobody asks

Should you send parts to a finisher that already did your job badly? Occasionally, yes — usually when the previous supplier had a hard time with the material and gave up. But the more common outcome is that we look at the parts, find the previous route was the problem, and quote something different. inflpanjlott.com would rather explain why the last finish failed than quietly put another layer on top of it.
